Photo Consent
I hereby grant permission for The Fishing School and news media covering The Fishing School to use my child’s photograph in videotapes, publications, motion pictures, recordings, social media posts, and all other events. I also consent to using my child’s written work in TFS publications. I hereby release The Fishing School’s programs from any forms of payment involving the current or future use of photographs, videotapes, or audio recordings for which they have performed.
This statement is hereby acknowledged and agreed upon by me, with the full understanding that any photographs, video, audio tapes, or work produced by the child while attending programs will be freely used by The Fishing School program for public display in either printed or electronic material to further the business interest of the program.

FERPA Consent
I hereby authorize and consent to the DCPS Office of the Chief of Staff, Office of the State Superintendent, and my child’s school to provide information concerning the education of my child to The Fishing School and the DC Office of Out of School Time Grants and Youth Outcomes (OST Office). I further authorize the release of educational records of my child for the enrolled school year to the parties listed above that include the following information: education transcripts, school/program enrollment information, universal student ID, address, demographic data, attendance data, credit history, grades, assessment data, and IEP information. This authorization and release shall remain effective from May 15, 2024, to August 15, 2025, and all information will be kept confidential. The Fishing School will use this information for internal evaluation purposes and to ensure the appropriate support is provided based on a child’s need to the best of our ability. By checking this box and submitting this enrollment form, 1) I acknowledge and understand that I have the opportunity to review the records to be disclosed and the right to challenge the contents of such records, and 2) I am at least 18 years of age or I am signing this document on behalf of my child because he/she is not 18 years of age.

SAYO Opt-Out
To find out how children and youth are doing, we will be using a questionnaire called the Survey of Academic and Youth Outcomes (SAYO).
The SAYO looks at youth’s participation in afterschool activities and measures healthy outcomes for children and youth. Your child may be invited to complete a brief afterschool survey. The survey will ask questions about what your child thinks of the program and about some of the ways he or she may be benefiting from attending the program.
The information your child provides will be extremely valuable in helping us to improve our program. All information collected will be kept confidential. Your child’s name will not appear on SAYO materials. All findings from the surveys will be combined and reported at the group level. No individual children or youth will be identified.
We do not see any risks to you or your child associated with participating. The information gathered will be used to improve our afterschool program and to generate greater support for afterschool programs in the community. Your participation is voluntary.
We encourage all parents to agree to allow their children to participate so that we can have the most complete information possible about how our program is affecting children and youth.
Para saber cómo les va a los niños y jóvenes, utilizamos un cuestionario llamado Encuesta de Resultados Académicos y Juveniles (SAYO).
La encuesta SAYO se centra en la participación de los alumnos en actividades extraescolares y evalúa los resultados positivos para niños y jóvenes. Es posible que invitemos a su hijo/a a rellenar una breve encuesta sobre actividades extraescolares. En la encuesta se le harán preguntas sobre lo que piensa su hijo/a del programa y sobre las ventajas que puede obtener de su participación en el mismo.
La información que proporcione su hijo/a será de gran utilidad para ayudarnos a mejorar nuestro programa. Toda la información recopilada es confidencial.
